Case Manager: ACT/SUD/Behavioral Health

Provides support and advocacy to clients and their families within the community to educate and promote healthy lifestyles and preventing reoccurring hospitalizations by monitoring, assessing, advocating, supporting and empowering members to improve their quality of life through maintaining stability in the community.

Education & Experience

• Bachelor’s degree in human services required.
• Basic Life Support (BLS) certification required
• One year of experience in treating or working with a person who is exhibiting mental illness, substance abuse, and/or co-occurring behaviors preferred.
• Valid Michigan driver’s license required
