Total Care Services, Inc. (TCS) is seeking a Case Manager for Adult Medical Day Care Services (MDCSW) to support individuals with complex medical and functional needs. In this role, you will help clients receive quality care in community-based settings rather than institutional environments.

Compensation & Benefits

• Base Salary: $60,000
• Quarterly Bonus: Up to $1,250 based on performance
• Public Service Loan Forgiveness (PSLF) eligible employer

About the Role

As a Case Manager – Medical Day Care Services, you will provide person-centered case management to individuals (age 16+) who meet nursing facility level of care but choose to live in the community.

Key Responsibilities

• Conduct assessments using Person-Centered Planning (PCP)
• Develop and manage individualized service plans
• Coordinate Adult Medical Day Care and community-based services
• Document all service coordination activities in LTSS Maryland daily
• Conduct in-person client visits
• Monitor service effectiveness and client outcomes
• Ensure compliance with program and regulatory requirements
• Maintain accurate and timely documentation

Qualifications

Required:

• Bachelor’s degree in Human Services or related field (or approved equivalent)
• Some experience in case management or service coordination
• Strong communication, documentation, and organizational skills
• Valid driver’s license, reliable transportation, and safe driving record
• Ability to travel locally throughout Maryland
• Must be at least 21 years old
• Ability to pass background checks
• Authorized to work in the United States

Preferred:

• Experience with Adult Medical Day Care or Medicaid Waiver programs
• Familiarity with LTSS Maryland system
• Experience working with individuals with complex medical or behavioral needs

Schedule

• Full-time, Monday through Friday
• Flexible based on client needs
