Case Manager Adult Psychiatric Rehabilitation Program

Job description:

POSITION SUMMARY: The psychiatric rehabilitation case manager provides services and functions as part of a team providing rehabilitative services to patients living in the community.

DUTIES AND RESPONSIBILITIES:

1. Provide rehabilitative services that may include service linkage, assessment and planning of services, patient advocacy, monitoring of service delivery, patient outreach, and documentation.

2. Provides crisis intervention services.

3. Provides liaison services for patients with various community agencies.

4. Functions as part of a treatment team to serve and meet the needs of patients.

5. Participates in the development and implementation of patient treatment and rehabilitation goals.

6. Provides transportation to patients as required.

7. Document all clinical activities in accordance with agency requirements.

8. Participates in 24-hour emergency coverage rotation and provides crisis intervention services.

9. Participates in ongoing clinical supervision according to agency policy.

10. Works schedule according to needs of agency.

11. Attend committee meetings as assigned.

12. Maintain a minimum caseload of 18 patients.

13. Perform other duties as required.

QUALIFICATIONS:

--- A.A. degree in a mental health field.

--- Bachelor's degree in a mental health field.

--- Master's degree from an accredited college or university in a mental health field.

--- Must possess a valid driver's license.

Pay: From $21.00 per hour
