We are seeking a motivated and compassionate Case Manager to join our 50-bed Assisted Living Facility. This position is responsible for admissions coordination, resident case management, and census growth through referral development and community outreach. The ideal candidate has healthcare admissions experience, strong relationship-building skills, and the ability to guide families through the placement process while supporting regulatory compliance. Sunday–Thursday schedule.
Key Responsibilities
- Conduct pre-admission assessments to determine appropriateness for assisted living
- Coordinate admissions from hospitals, rehabilitation centers, SNFs, and the community
- Review and collect admission documentation
- Guide families through the tour → application → admission process
- Develop and update resident service plans in coordination with facility Administrator & Nurse
- Participate in care conferences and transition planning
- Serve as liaison between residents, families, nursing staff, physicians, and outside providers
- Develop and maintain referral relationships with:
- Hospital case managers & social workers
- Home health & hospice agencies
- Community organizations & senior resources
- Conduct facility tours and follow-ups as needed
- Track leads, referrals, and conversion rates
- Represent the facility at community events and outreach activities
- Collaborate with the Administrator on occupancy and census goals
Compliance & Documentation
- Ensure admissions documentation complies with state DOH regulations
- Maintain accurate resident records and assessments
- Support survey readiness related to admissions and resident services
Qualifications
- Experience in assisted living, healthcare admissions, case management, or healthcare marketing
- Knowledge of discharge planning and care coordination
- Strong communication and organizational skills
- Ability to work independently and manage multiple referrals
- computer proficiency
- Associates degree with three years of experience, OR bachelor’s degree in healthcare, social work, or related field with one year of experience
Job Type: Part-time
Pay: $52,000.00 per year
Expected hours: No more than 25 per week
