Compensation: $22–$25 per hour, depending on experience
Position Overview
Stone Healing Treatment Center is seeking a compassionate and organized Case Manager / Qualified Mental Health Specialist (QMHS) certified through CCAPP to provide case management services and facilitate mental health psychoeducational groups in our PHP and IOP programs.
This role supports clients receiving structured outpatient mental health treatment by assisting with care coordination, discharge planning, life skills development, and community resource connection. The Case Manager will also facilitate structured mental health groups focused on psychoeducation, coping skills, and recovery support.
The ideal candidate is organized, able to build rapport with clients, and comfortable leading groups while working collaboratively within a multidisciplinary treatment team.
Key ResponsibilitiesCase Management Services
- Provide individual case management services to clients in PHP and IOP programs.
- Assist clients with care coordination, discharge planning, and aftercare referrals.
- Connect clients with community resources, including outpatient providers, psychiatric services, housing support, employment resources, and support groups.
- Coordinate with therapists, psychiatrists, and treatment team members to support client progress.
- Assist clients in developing life skills, recovery plans, and personal goals.
- Maintain communication with outside providers and referral sources when clinically appropriate.
Group Facilitation
- Facilitate psychoeducational and skills-based mental health groups within PHP and IOP programming.
- Lead groups on topics such as:
- Coping skills and emotional regulation
- Stress and anxiety management
- Mental health education
- Communication and relationship skills
- Relapse prevention and recovery support
- Life skills and goal setting
- Maintain a structured and supportive group environment that promotes engagement and participation.
Documentation & Compliance
- Complete timely and accurate documentation in the electronic health record (EHR), including:
- Case management notes
- Group notes
- Discharge planning documentation
- Ensure documentation meets insurance, regulatory, and program requirements.
- Participate in clinical team meetings and case reviews as needed.
QualificationsEducation & Certification
- Qualified Mental Health Specialist (QMHS) through CCAPP (or willing to get immediately)
- Bachelor’s degree in psychology, social work, counseling, or related behavioral health field preferred
Experience
Preferred experience in:
- Mental health treatment programs
- PHP or IOP settings
- Group facilitation
- Case management or care coordination

Pay: $22.00 - $25.00 per hour
