Case Manager - Beacon of Home

Duties

  • Conduct thorough patient assessments, including medical history review, physiological evaluation, and psychosocial factors to develop personalized care plans.
  • Coordinate discharge planning by collaborating with multidisciplinary teams, ensuring timely and appropriate patient discharges from hospital or clinic settings.
  • Manage utilization review and utilization management processes to optimize resource use while maintaining quality standards.
  • Facilitate diagnostic evaluations and coordinate post-acute care services such as home health, rehabilitation, or outpatient clinics.
  • Maintain accurate and confidential medical records in compliance with HIPAA regulations; document all case management activities meticulously.
  • Provide patient education and support regarding treatment options, medication adherence, and community resources to empower informed decision-making.
  • Liaise with insurance providers and managed care organizations to authorize services and streamline authorization processes.

Skills

  • Proven experience in hospital settings with expertise in Level I Trauma Centers, Level II trauma centers, or similar acute care environments.
  • Strong background in hospital medicine, emergency medicine, ICU, PICU (Pediatric Intensive Care Unit), or behavioral health settings.
  • Knowledge of medical terminology, physiology, diagnostic evaluation techniques, and inpatient/outpatient care workflows.
  • Familiarity with HIPAA compliance, medical records management, intake procedures, discharge planning, and utilization management protocols.
  • Excellent patient service skills with the ability to perform comprehensive patient assessments and coordinate complex care plans effectively.
  • Experience in addiction counseling or mental health counseling is a plus; familiarity with post-acute care pathways highly desirable.

Pay: From $45,000.00 per year
