Case Manager-Bilingual preferred

Provide effective case management for children and families residing in Family Services programs including the David Jon Louison, the Family Life Center, and our Bolton Place Center. Case Management includes monitoring compliance with the Executive Office of Housing and Livable Communities self-sufficiency plan, assessment of client strengths/needs; referrals and linkages to community based services; preparation to return to an independent living situation, and working with family in community after they have been rapidly re-housed.

Requirements:

EDUCATION/EXPERIENCE/PHYSICAL REQUIREMENTS:

  • Degree in Human Services or related degree preferred.
  • Minimum of five years experiencing in human services or residential setting.
  • Valid Driver's License, proof of insurance and driving record.
  • Experience with in crisis youth and families
  • Experience w/Microsoft office suite. Suite includes word, excel, access and PowerPoint.
  • Additional skills should include grammar, punctuation, spelling and business writing.
  • Ability to lift 50 lbs on occasion.
  • Must be able to walk, sit, stand, bend, or stoop, and walk up and down stairs as needed during the work day.
