The Dane County Human Services Department, Disability and Aging Services Division, Children's Long Term Support (CLTS) Unit, in Madison, Wisconsin, is recruiting for a full-time (1.0 FTE) Case Manager (Bilingual - Spanish) position. This position is designated as bilingual and candidates will be tested in Spanish and English during the recruitment process. Employees who work in bilingual designated positions receive an additional $2.00 per hour (not reflected in the above hourly rate).
The Eligibility List created may be used to fill future vacancies over the next six (6) months.
Note to applicants – for the 2026 budget year, there is a 1% reduction in wages (which is already reflected in the hourly wage above), an additional 21 holiday hours, and continued earning of longevity and merit steps.

DEFINITION
Case Managers work closely with children with disabilities, families, guardians, service providers and natural supports to coordinate support services necessary for children to live in their family home, attend their neighborhood school, work and participate as fully as possible in community life. Promote self-determination, community belonging, valued social roles, the sharing of ordinary places, opportunities for contribution, and choice while seeking to ensure health and safety.
EXAMPLES OF DUTIES
Establish on-going relationships and partnerships with children, their families, guardians, service providers and natural supports. Perform individualized assessments through review of historical information, interviews and in-person observation with the individual, their family, friends, advocates, teachers, service providers, medical professionals and other team members. Act as team leader, facilitating interdisciplinary meetings, coordinating services and seeking outside resources and consultation as needed. Facilitate, write and distribute annual individualized service plans, and court reports as required, outlining the support an individual needs to live, attend school and/or work successfully in the community and how it will be funded. Implement the individualized plans to the maximum degree possible by ensuring all available social and financial resources are known and explored, including family relationships, community connections and friendships, disability services, and other community resources. Determine gaps in the existing unpaid and paid support network then work to eliminate these gaps, developing new supports and seeking funding when needed.
Basic understanding of human development and behavior and the awareness of how developmental disabilities and sensory-motor differences may affect this. Knowledge of current human service philosophies and approaches such as self- determination, person-centered planning, community inclusion, trauma informed care and asset based community development. Knowledge of, and experience applying case management methods, policies, procedures and treatment techniques. Ability to develop positive, long-term relationships with people, understand their concerns and act to address them in a timely, respectful and effective manner. Ability to quickly acquire and apply knowledge of laws, regulations, policies and procedures relating to the target population. Knowledge of and ability to access community resources. Ability to prioritize, calmly manage and resolve urgent or crisis situations. Capacity to organize large amounts of information and manage complex situations to achieve the most effective individual and programmatic outcomes. Ability to be decisive, consistent and diplomatic. Excellent personal, written and verbal communication skills. Ability to follow oral and written instructions, autonomously meet deadlines, participate in and appropriately use supervision. Ability to participate effectively as a team-member. Ability to engage and work with people from diverse ethnic and cultural backgrounds with understanding and respect for the individuals and their differences. Knowledge of computer based applications including word, excel and other related software. Ability to use a computer to generate reports, complete forms, and document case activity. Ability to prepare and maintain complex records and reports.
