Case Manager - Care Coordination

DISC Village, a non-profit community-based agency, has been in operation for over 50 years and is one of the largest and most comprehensive prevention, intervention and treatment agencies in Florida. Through the years, DISC Village has become recognized as a leader in the state for initiating, managing and coordinating substance abuse prevention/intervention, family welfare and delinquency programs in Leon County and North Florida.

Essential Responsibilities:

  • Identify and engage individuals eligible for Care Coordination services through targeted outreach.
  • Serve as the primary point of contact, coordinating care among providers, families, and other involved parties.
  • Maintain ongoing contact with individuals and connect them to appropriate treatment, support, and recovery services.
  • Complete biopsychosocial and needs assessments in accordance with the Wraparound model.
  • Document assessment findings, referrals, services provided, and progress in a timely and accurate manner.
  • Prepare and submit required reports and documentation to the Supervisor and Qualified Professional within established deadlines.
  • Collaborate with internal and external partners to support continuity of care and positive outcomes.
  • Travel within the service area as needed to provide outreach and care coordination services.

Qualifications:

  • Bachelor’s degree in Social Work, Psychology, Human Services, Counseling, or a related field preferred.
  • Experience working with individuals experiencing behavioral health, substance use, or co-occurring disorders preferred.
  • Knowledge of care coordination, case management, recovery support services, and community resources.
  • Ability to conduct biopsychosocial assessments and develop individualized service plans.
  • Strong interpersonal, communication, and organizational skills. Ability to work effectively with individuals, families, service providers, and community partners.
  • Proficient in documentation, data entry, and maintaining accurate client records.
  • Knowledge of the Wraparound model and trauma-informed practices preferred.
  • Ability to manage multiple priorities and meet established deadlines.
  • Valid driver’s license, reliable transportation, and ability to travel within the service area.
