Join our team as a Case Manager with the Community Housing Assistance Program (CHAP) and help individuals experiencing homelessness access resources, stabilize their lives, and move toward permanent housing.

Key Responsibilities:

Build supportive relationships with participants experiencing homelessness.

Provide supportive counseling and daily living skills training.

Connect participants to community resources and assist with discharge planning.

Provide crisis intervention and assist with meeting basic needs.

Conduct home visits and provide transportation when needed.

Maintain documentation and case records.

Promote a culture of inclusion and belonging.
Qualifications:

Associate or Bachelor’s degree in Human Services, Social Work, or a related field preferred (equivalent experience considered).

Experience working with diverse or high-needs populations preferred.

Valid NYS driver’s license and access to a reliable vehicle required.
