The Case Manager (Client Acquisition Specialist) is responsible for driving company growth by bringing in new clients/patients. This role is ideal for a self-starter who thrives in a fast-paced environment, enjoys networking, and is passionate about connecting individuals with essential home care services.

Responsibilities

• Proactively identify and engage with hospitals, clinics, physicians, and community organizations to generate new patient referrals.
• Actively recruit and onboard new clients/patients for home health care services
• Build and maintain relationships with hospitals, rehabilitation centers, social workers, discharge planners, and community organizations
• Conduct outreach and marketing efforts in the community to generate referrals
• Educate potential clients and families on available services and benefits
• Track leads, referrals, and conversion rates to meet weekly and monthly growth targets
• Collaborate with internal team to ensure high-quality service delivery and client satisfaction
• Maintain strict compliance with HIPAA regulations by safeguarding patient information during all stages of case management and documentation.
• Track referral progress through detailed documentation of intake activities, patient service status, and discharge planning updates.

Qualifications

• Excellent communication skills for engaging with patients, families, healthcare providers, and referral sources effectively.
• Proven ability to meet or exceed growth or sales targets
• HS Diploma/College Degree is NOT required but it is preferred.
• Knowledge of Medicaid/Medicare and home care services is a plus
• Valid driver’s license and reliable transportation preferred

Pay: $800.00 - $3,000.00 per week
