Summary
Responsible for managing multiple aspects related to client services (orientation and introduction to the program, discharge planning, and oversight of client needs etc.). This person can direct him or herself, has a strong therapeutic presentation, is organized, and has excellent time management skills and the ability to collaborate with team members involved in client care.
DUTIES/EXPECTATIONS
- Responsible for the client discharge plans.
- Responsible for the customer service aspect of each client’s introduction to the program, including the facilitation of a smooth transition from admission to discharge or step down.
- Provider oversight of client specific needs such as needed medical appointments, FMLA paperwork, COBs and any ancillary services that may be clinically indicated or beneficial.
- Help provide oversight by addressing client needs indicated on the admissions summary.
- Work in partnership with the clinical team and Outreach to collaborate and give resources and recommendations at discharge to help build business relationships with outside providers and professionals.
QUALIFICATIONS
- Bachelor’s degree in related field preferred.
- Two years of experience preferred
- Ability to read and interpret reports and other documentation.
- Ability to write reports and effectively present information and respond to questions from staff, families, and referral sources.
- This position requires use of “Handle with Care” protective hold procedures as a last resort for protection of residents and staff.
