Case Manager - Community Support Services

Southwestern Behavioral Healthcare is currently seeking a Full-Time Case Manager to add to our team of professionals. The Case Manager will provide community-based case management, skills training & development, and other treatment services to an adult outpatient population.

WHAT IS THIS POSITION RESPONSIBLE FOR?
  • Conduct case management, skills training & development, and other treatment services according to Southwestern policies.
  • Provide case management services to adults assigned to Outpatient Services including, but not limited to communicating with all parties involved in the client’s treatment; identifying treatment needs and facilitating all necessary referrals; coordinating and attending collateral meetings with all parties involved in the treatment process.
  • Provide emergency counseling and crisis intervention when necessary.
  • Maintain appropriate records of services to reflect patient progress and document treatment plans and goals.
  • Communicate professionally, make referrals, and coordinate services with community agencies.
  • Must meet efficiency standards which include chart management and patient contact standards
WHAT'S REQUIRED FOR THIS POSITION?
  • Bachelor's degree in Social Work, Psychology, Sociology, Criminal Justice, or a related Human Service field.
  • Knowledge of available community resources is preferred.
  • Previous work with the SMI population is highly preferred.
  • Demonstrated proficiency with interpersonal and writing skills.
  • Excellent communication, time management, and organizational skills.
  • Must have valid driver's license, auto insurance that meets the Indiana State minimum coverage requirement, and qualify for Company's vehicle liability insurance.
  • Candidates must pass required background checks including county/state checks, sex offender registry check, and drug screen.
