Case Manager (Detox) - Substance Use

Overview
We are seeking a compassionate and dedicated case manager to join our detox team. This role is essential in providing support and guidance to individuals undergoing detoxification. The ideal candidate will possess a strong background in various therapeutic modalities and demonstrate a commitment to helping clients achieve their recovery goals.

Responsibilities

• Perform comprehensive patient assessments to develop tailored treatment plans.
• Facilitate crisis management and provide direct support to clients experiencing acute distress.
• Maintain accurate medical documentation, including ICD-9 and ICD-10 coding, in compliance with HIPAA regulations.
• Collaborate with multidisciplinary teams to ensure holistic patient care.
• Provide education and resources related to addiction counseling, mental health, and behavioral health issues.
• Participate in case management activities, ensuring continuity of care for clients transitioning from inpatient to outpatient services.

• Proficiency in case management services
• Case management, organization, and multitasking
• Knowledge of FMLA and short-term disability processes
• Clear communication with clients, employers, and providers
• Employer coordination and return-to-work planning
• Familiarity with legal documentation, compliance, and court procedures
• Client advocacy and needs assessment
• Attention to detail and accurate recordkeeping
• Proficiency in Microsoft Office and case management systems

Job Type: Part-time

Pay: From $28.00 per hour
