Case Manager - Developmental Disabilities

Cumberland Mountain CSB is seeking a professional interested in providing case management services to developmentally disabled individuals. Duties include writing individual service plans, monitoring progress toward specified goals, advocating for needed resources and coordinating multi-agency involvement. Must work cooperatively with community agencies and state facilities to serve individuals with developmental disabilities who are in need of case management services. Includes traveling to homes, community events and state facilities to provide case management and discharge follow-up services.

Extensive documentation of services necessary to meet regulatory guidelines.
