Responsibilities:
- Responsible for handling all aspects of case management.
- Attend court to inform potential participants about program details/requirements
- Perform needs assessment for individualized case plan development for all new participants
- Revise case plans as needed for participant progress
- Frequent contact consistent with program requirements with all participants to ensure program progress
- Refer participants to appropriate services based on participants’ individualized needs
- Collaborate with community providers to ensure continuity and quality of care for participants
- Resolve emergency problems in crisis situations
- Assist clients with psychoeducational and life skills
- Provide progress reports to Prosecutor and Court
- Use of motivational interviewing techniques and a trauma-informed approach in all case management functions
- Practice cultural competence in case handling with awareness and respect for diversity
- Document all participant interactions and progress by tracking and maintaining timely participant notes
- Maintain confidentiality of participants’ information
- Skillful management of case load
- Participate as a team member in fostering innovative programming
- Perform other duties as assigned
Skills:
- Bachelor's degree in social work or a related field from an accredited university. Master's degree preferred.
- Experience in case management preferred
- Proficient in Microsoft Word and Excel
Pay: $49,500.00 - $60,000.00 per year
