Responsibilities:

  • Responsible for handling all aspects of case management.
  • Attend court to inform potential participants about program details/requirements
  • Perform needs assessment for individualized case plan development for all new participants
  • Revise case plans as needed for participant progress
  • Frequent contact consistent with program requirements with all participants to ensure program progress
  • Refer participants to appropriate services based on participants’ individualized needs
  • Collaborate with community providers to ensure continuity and quality of care for participants
  • Resolve emergency problems in crisis situations
  • Assist clients with psychoeducational and life skills
  • Provide progress reports to Prosecutor and Court
  • Use of motivational interviewing techniques and a trauma-informed approach in all case management functions
  • Practice cultural competence in case handling with awareness and respect for diversity
  • Document all participant interactions and progress by tracking and maintaining timely participant notes
  • Maintain confidentiality of participants’ information
  • Skillful management of case load
  • Participate as a team member in fostering innovative programming
  • Perform other duties as assigned

Skills:

- Bachelor's degree in social work or a related field from an accredited university. Master's degree preferred.
- Experience in case management preferred
- Proficient in Microsoft Word and Excel

Pay: $49,500.00 - $60,000.00 per year
