Emergency Shelter Case Manager

Job Summary

The Shelter Case Manager is responsible for the daily case management of the residents who are staying at the Chayah House Emergency Shelter. The primary focus of this work is to connect residents with resources and services to assist them in stabilizing their homeless, mental health and primary health situation. Through case management support, the individuals experiencing homelessness are aided in resolving their immediate basic needs and establish housing plans. The Shelter Case Manager will form relationships with area resources, other shelters, and landlords to secure stable housing options for shelter.

  • Consistently report to work on time prepared to perform duties of position.
  • A strong work ethic having the needs of the organization and the clients as the primary priority.
  • Ambassador for Loaves and Fishes Ministries along with all employees, and responsible for the marketing and promoting the organization and its program both internally and externally.
  • Lead in being a heartfelt team member of Loaves and Fishes Ministries.
  • Display Christ-like qualities in attitudes, work, interpersonal relationship and action.
  • Assist in communicating our mission and core values in word and deed inside and outside of the program.
  • Help plan program events such as fundraisers, graduations, and other special events.

Duties and Responsibilities:

· Be a consistent office presence to help provide safety, structure and support for clients, volunteers, coworkers from other agencies and potential applicants.

● Meet with shelter residents at least weekly to establish goals and action steps.

● Connect residents with community resources.

● Filing confidential client documentation.

● Help residents create support systems and participate in the community as they desire.

● Maintain relationships with service providers, volunteers, staff and community members.

● Maintain regular contact with the supervisor and shelter support staff to apprise them of the residents’ progress and to assist in the resolutions of potential problems.

● Assist families and individuals with a move out plan.

● Work with clients on a one-on-one basis to provide education, guidance and referrals tailored to each client’s unique circumstances and create housing stability plans.

● Assist and coach clients with job searches, job pavements, interviews preparation, resume modification and promote job retention.

● Work with residents to obtain housing stability with permanent housing.

● Must complete the HMIS training and gain access to the HMIS system.

● Enter information into the HMIS system in a timely manner.

● Upload client documentation into the agency database and the HMIS system.

● Maintain accurate and complete resident files per grant guidelines.

Qualifications, Experience and Abilities

· High School graduate with some college preferred (experience can substitute for education)

· Experience working with families, homelessness, mental health, and/or substance abuse that reflects an understanding of the causes of homelessness is strongly preferred.

· Great listening skills

· Excellent written and verbal communication skills

· Personal relations skills with a desire to work with persons in crisis and vulnerable individuals; yet complying with .

· Organization and documentation skills

· Work with little or no supervision – takes initiative

· Computer proficiency in Microsoft Office programs

· Ability to establish rapport

· Ability to motivate others towards achieving goals

· Ability to work independently with strong sense of focus, task-oriented, nonjudgmental, open personal qualities

· A strong sense of and respect for confidentiality involving both clients and fellow employees

· Demonstrate high ethical behavior and integrity in working with clientele, co-workers and supervisors

· Knowledge of community resources

· Flexible schedule

· Valid Colorado Driver’s License

· All staff must undergo a background check.

· Must be able to maintain confidentiality of information.

Physical Requirements:

● Ability to operate a variety of office equipment including computer terminal, telephone, fax machine, calculator/adding machine, photocopier.

● Ability to coordinate eyes, hands, feet and limbs performing semi-skilled movements such as data entry.

● Ability to exert sustained moderate physical effort in light work, typically involving some combination of stooping, kneeling, crouching and crawling, lifting, carrying, pushing and pulling. Ability to sustain prolonged visual concentration.

● Ability to recognize and identify similarities or differences between characteristics of colors and sounds associated with job-related objects, materials and tasks.

Pay: $35,900.00 - $36,798.00 per year
