Duties
- Develop individualized care plans tailored to each client’s needs, focusing on behavioral health, addiction recovery, lifeskill, productive self efficient member of society and workforce development goals
- Conduct thorough assessments to identify client strengths, challenges, and service needs using motivational interviewing techniques and document
- Coordinate with community agencies to ensure clients receive appropriate services such as addiction counseling, mental health support, and Section 8 housing assistance
- Provide crisis management by responding swiftly to urgent situations with effective de-escalation strategies and referral to emergency services when necessary
- Support clients in navigating the criminal justice system, including understanding Section 8 regulations and other legal requirements
- Facilitate workshops or group sessions on behavioral health topics, life skills, and workforce development strategies
- Maintain detailed documentation of client interactions, progress notes, and service plans in compliance with organizational policies
Qualifications
- Creative skill sets in behavioral health, or related fields with a focus on male populations preferred
- Knowledge of community resources, housing options and workforce development programs highly desirable
- Strong skills in communication and crisis management techniques
- Familiarity with motivational interviewing methods to promote client engagement and behavior change
- Ability to work effectively within a multidisciplinary team while maintaining a positive, energetic attitude
- Excellent communication skills with an emphasis on transparency and active listening
- Reliable vehicle, valid drivers license and insurance required
