Case Manager for Pillars Program

The Case Manager, under the direct supervision of the Program Director, will provide case management with particular focus on guidance and goal measurement to the residents of the House of Hope Pillars Program as well as Outreach Participants. The Case Manager will use standard social work practice guidelines and adhere to the Social Work Code of Ethics in all work with program participants, including but not limited to assessment, documentation, development of individual service plans, and discharge plans.

Requirements:

  • BA in human services or equivalent experience required.
  • One year of professional experience providing services specifically to at risk youth preferred.
  • Must possess flexibility as well as understand healthy boundaries.
  • Must be able to work independently as well as part of a team.
  • Familiarity with Windows, Microsoft programs, Outlook.
  • Working knowledge of Waterloo/Cedar Falls resources and community agencies.
  • Valid Driver’s License and proof of automobile insurance

Supervisor: House of Hope Program Director

Duties

  • Meets with each program participant weekly, addressing strengths and needs, and progress toward goal attainment.
  • Acts as liaison to ensure participation in other House of Hope, Aftercare, and Health programs.
  • Uses a Trauma Informed Care model to partner with each program participant in writing a usable, applicable case plan, forming SMART goals, and conducting weekly sessions.
  • Ensures cleanliness and communicates maintenance needs of the Pillars House.
  • Articulates and models the House of Hope mission, vision and values to visitors, volunteers, staff, stakeholders, and program residents.
  • Speaks to groups on site and off site to educate about the program as well as to recruit potential residents.
  • Completes accurate documentation of all session notes, program participant contacts and collateral communication within 72 hours of service provision.
  • Assists with scheduling, organizing and conducting weekly group sessions with program participants as well as fundraisers and special/Holiday activities.
  • Together with the Program Director, screens applicants, conducts intake interviews, and follows admission procedures to admit program participants.
  • Communicates in a timely manner any concerns regarding program participants or programming to the House of Hope Executive Director and meets to discuss these concerns as needed.
  • Is available to address client concerns after hours in case of emergent situations, alerting the Program Director of all concerns and consulting as needed. Shares on call schedule with the Case Management Team.
  • Greets the public, answers phones, and assists donors as needed including unloading and sorting donated items.
  • Other duties as assigned.

Work Environment:

  • Ability to work in a chaotic environment with loud noise levels.
  • Able to work varied hours which include nights, weekends and holidays.

Job Type: Full-time

Pay: $40,000.00 - $50,000.00 per year
