Case Manager for Veteran Transitional Housing Program- Paid Training & PTO

Job Description

The Transitional Housing Case Manager will:

· Facilitate the delivery of appropriate supportive services to veterans who are experiencing homelessness and living in a transitional housing environment.

· Conduct needs-based assessments, assist veterans with connecting to public benefits, VA benefits, housing resources and other community resources as deemedappropriate.

· Work alongside the Program Manager to ensure treatment plans are being implemented effectively and the veterans are connected to services that will help them increase self-sufficiency and promote self-determination.

· Assist the veterans with identifying and applying for permanent housing.

· Act as an advocate for the veterans with both landlords and other community partners.

Knowledge and Skills:

· Good written, oral and interpersonal communication skills

· Strong computer skills

· Strong time-management, prioritization and critical thinking skills

· Ability to work both independently and in a team-oriented, collaborative environment

· Fundamental understanding of Trauma informed care and a strengths-based service approach

· Ability to maintain accurate and detailed documentation regarding client progress and interactions

· Ability to be flexible and adaptive to unexpected challenges and changes

· Ability to present themselves professionally and maintain good working relationships with landlords and other community partners

Required Qualifications:

· High school diploma or GED

· Working knowledge of fundamental case management duties

· Able to take a rotating on-call phone in 1-week increments

· Clean motor vehicle record and background check

Preferred Qualifications:

· 1-2 years of case management experience with vulnerable populations

· Associates or bachelor’s degree in social work, social sciences, public administration, psychology or related field

· Veteran preferred

Compensation and Benefits:

· $21-23/hour -based off experience

· Paid training to become a Certified Case Manager (will receive a $0.50 pay increase upon completion)

Pay: $21.00 - $23.00 per hour
