Case Manager for veterans

Our agency is seeking a compassionate, organized, and mission-driven professional to serve as a Supportive Services for Veteran Families (SSVF) Case Manager under the U.S. Department of Veterans Affairs SSVF program.

This position provides housing stability and supportive services to very low-income Veterans and their families who are homeless or at risk of homelessness.

Position Summary

The SSVF Case Manager provides intensive case management and supportive services designed to promote long-term housing stability. Services include assessment, housing navigation, service coordination, and connection to community resources.

Essential Responsibilities

  • Conduct eligibility screening and intake in accordance with U.S. Department of Veterans Affairs SSVF guidelines
  • Complete comprehensive assessments and develop individualized housing stability plans
  • Assist Veterans in securing and maintaining permanent housing
  • Coordinate Temporary Financial Assistance (TFA) as appropriate
  • Connect participants to employment services, healthcare, behavioral health, and benefits
  • Maintain accurate documentation and data entry in HMIS
  • Conduct community-based outreach
  • Collaborate with landlords and community partners

Qualifications

  • Associate or Bachelor’s degree in social work, human services, or related field (preferred)
  • Experience working with Veterans, homeless populations, or housing programs preferred
  • Knowledge of community resources and housing systems
  • Strong organizational and documentation skills
  • Ability to work independently and manage a flexible schedule
  • Valid driver’s license with clean record

A minimum degree required is an Associate’s Degree Human Services or a BA in Social Work or related field of study.

Job Type: Full-time
