Case Manager - Foster Care

Have Haven Child Placing Agency is seeking a dedicated and compassionate Case Manager to join our team in providing exceptional services for foster children and families. As a Case Manager, you will play a pivotal role in ensuring the safety, well-being, and stability of children in foster care by working closely with foster families, biological families, and other stakeholders to support successful placements.

Key Responsibilities

  • Manage a caseload of foster children and families, conducting regular home visits and assessments.
  • Develop and implement individualized service plans (ISPs) tailored to each child's needs.
  • Collaborate with foster families, biological families, medical professionals, schools, and other community resources.
  • Monitor placements to ensure compliance with Texas Minimum Standards and agency policies.
  • Provide crisis intervention, guidance, and support to foster families as needed.
  • Maintain accurate and up-to-date documentation in accordance with state and agency guidelines.
  • Participate in ongoing training and professional development.
  • Other related duties as assigned.

Qualifications

  • Education: Bachelor’s degree in Social Work, Psychology, or a related field (Master’s degree preferred).
  • Experience: Minimum of 1-2 years in case management or child welfare services preferred.
  • Familiarity with Texas DFPS Minimum Standards and child placing agency operations is a plus.
  • Strong organizational, communication, and interpersonal skills.
  • Ability to handle sensitive and confidential information with professionalism.
  • Proficiency in Microsoft Office and case management software.
  • Must have a valid driver’s license and reliable transportation.
