Case Manager - Foster Care

Case Manager – Foster Care (Full‑Time)

Location: Tucker Office (reports to Foster Care Supervisor) or Gainesville Office (reports to Program Manager)

Position Type: Full‑Time (Hybrid)

Position Summary

The Case Manager – Foster Care is responsible for the placement, ongoing support, and case management of children in foster homes. This role ensures the safety, well‑being, and permanency planning of children in care while providing supervision, training, and support to foster parents. The position also includes casework services for birth families and coordination with internal and external partners.

Key Responsibilities

  • Team Collaboration — Actively participate as a contributing member of the agency’s foster care team.

  • Training & Professional Development — Attend required trainings, consultations, and conferences as directed by the Foster Care Supervisor or Program Manager.

  • Organizational Communication — Maintain organized workflow and keep leadership informed of client needs and priorities.

  • Compliance & Documentation — Meet all compliance deadlines for foster youth and foster parent records, including ORCC, DHS, and COA requirements.

  • Financial & RBWO Requirements — Submit monthly financial vouchers and RBWO per diem documentation on schedule.

  • Service & Permanency Planning — Coordinate and participate in service planning and permanency efforts for all assigned families.

  • Quality Assurance — Participate in quarterly peer monitoring of children’s and foster parents’ records.

  • Monthly Visits — Conduct required monthly visits to foster homes and available birth parents.

  • Outcome Tracking — Support outcome planning and maintain data to measure program effectiveness.

  • GA SCORE Data Entry — Enter required data into GA SCORE and meet monthly deadlines.

  • Annual Reporting — Collect and enter data used for the agency’s annual report.

  • Confidentiality — Maintain strict confidentiality in all casework and communication.

  • Policy Adherence — Follow all agency personnel and program policies.

  • Public Relations — Represent the agency professionally in all community interactions.

  • Quality Assurance Participation — Support agency quality assurance activities.

  • Other Duties — Perform additional responsibilities as assigned by the Foster Care Supervisor or Program Manager.

Requirements

  • Bachelor’s Degree — Minimum of a bachelor’s degree from an accredited college or university in a human services or behavioral science field.

  • Experience with Children and Families — At least two years of direct experience working with children and families.

  • Reliable Transportation — Must own, operate, and maintain a personal vehicle in good working condition; mileage reimbursement is provided for agency‑related travel.

  • Flexible Schedule — Position requires some evening and weekend hours, as well as participation in after‑hours on‑call rotation.

  • Computer Literacy — Proficiency in word processing and Excel spreadsheets.

  • Organization & Detail Orientation — Strong organizational skills and attention to detail.

  • Communication Skills — Effective verbal and written communication abilities.

Work Environment

This role is based at a Wellroot‑assigned office (Tucker, GA) and involves work both in an office setting and across the agency campus. The environment is fast‑paced and involves direct interaction with vulnerable clients. A flexible schedule including evenings and weekends is required. The position may require lifting up to 25 pounds. Approximately 60% of the workday involves sitting at a computer.
