Case Manager (Foster Care & In-Home Services) Bachelor's Degree Required

Seraaj Family Homes, Inc. is seeking Case Managers to join our growing team based in the Brewton, Monroeville, Camden area. This position will provide services to clients in Escambia, Wilcock, Clark, Monroe and surrounding areas. ***This position requires a Bachelor's Degree in Social Work, Psychology, Criminal Justice or a related field with a minimumof 1 year experience.***

This position is primarily responsible for (not all-inclusive):

• Creating and maintaining a positive rapport with clients.
• Providing services in the homes of the foster parents/clients and other appropriate settings by visiting clients at least 2 times per week, more depending on the client’s needs
• Ensuring children and families receive required services per their treatment plan and individualized service plan (ISP)
• Interacting and physically engaging with children and youth in age-appropriate physical activities aligned with their treatment goals
• Assessing and advocating for options and services to meet the client’s needs
• Educating the client and his/her treatment team on the programs and options available to meet the client's needs.
• Assisting in transporting the client to their appointments
• Connecting clients to community resources based on their individual or family needs
• Accurately and timely entering a summary of provided services (session notes) in the web-based system, per policy
• Being accessible to children, families, and internal & external customers when needed and available to report on-site in the event of a crisis
• Performing other assigned duties

Job Type: Full-time

Pay: $16.00 per hour
