Case Manager (F/T, $50K + Benefits)

Schedule: In-Person/On-Site, 40-hour work week, primarily day shift, 8am-4pm or 9am-5pm Monday-Friday, but may also work an adjusted schedule one day per week to 6pm to meet with those guests who are employed.

Located in rural Ridgely, Maryland, Martin's House & Barn was founded in 1983 by the Benedictine Nuns. We offer an emergency family shelter, a community food pantry, and a community thrift store (the Barn). We are "A Community of Hospitality and Hope." We are community-centric, guest-focused, and empowerment-based. Our guests (clients/residents) have their own room, private 1/2/ bath, shared tub/shower with one other family, communal kitchen and living room, and their own dining table. Guests receive intensive case management, housing assistance, and connection to community resources.

Position Summary

The Case Manager (CM) works as part of the Martin's House & Barn homelessness and housing services team to assist and support guests (clients) to address barriers, identify resources to address them, and provides a variety of individual and family support services that meet immediate short-term needs and longer-term goals as guests (clients) strive toward personal and financial self-sufficiency. The CM's focus is on compassionate, trauma-informed, strengths-based services to assist individuals to secure stable housing, navigate barriers, learn to advocate for themselves and their families, and strengthen community partnerships.

Responsibilities, including, but are not limited to:

  • Conduct weekly case management meetings with all shelter guests (clients), and Rapid Rehousing Program participants.
  • In partnership with participants, develop Individualized Housing Stability Plans (IHSP) and monitor progress.
  • Coordinate weekly case conferences to receive updates on each guest and determine their needs.
  • Work with guests (clients) to coordinate transportation needs.
  • Utilize a trauma-informed, strengths-based approach in working with guests (clients) to reduce barriers, achieve short- and long-term goals, and secure and maintain permanent housing.
  • Maintain accurate data, files, case notes and other materials electronically (HMIS) and hard copies, in accordance with established methods and procedures and grant requirements.
  • Conduct telephone intake screenings and refer to community resources, as appropriate.
  • Attend regularly scheduled team and community meetings.
  • Coordinate with a variety of outside service providers to connect guests (clients) with available community resources.
  • Conduct all functions in accordance with funding agency guidelines, policies and procedures
  • Other duties as assigned.

Knowledge, Skills & Abilities

  • Knowledge and understanding of the needs and barriers associated with individuals experiencing homelessness, and formerly homeless individuals.
  • Familiar with Trauma-Informed Care, and Strengths/Empowerment-based models.
  • Familiar with low barrier shelters and Housing First.
  • Acceptance and understanding of individuals from diverse backgrounds with urgent and multiple barriers to self-sufficiency.
  • Ability to quickly develop a rapport, inspire, motivate, and coach guests (clients) towards achieving their goals.
  • Maintain professional boundaries when working with guests (clients).
  • Works well in a team and independently.
  • Excellent verbal and written communication skills.
  • Organized, detail-oriented, and can coordinate multiple demands in a outcomes-based environment.
  • Ability to write standardized clear and concise case notes.
  • Knowledge of Microsoft Word, Excel, PowerPoint, and Publisher.

QUALIFICATIONS

  • Bachelor's degree in Social Work, Psychology, or similar discipline desirable. MSW highly desirable.
  • Experience may be substituted for education on a year-for-year basis, up to two years.
  • 3+ years' experience in case management, preferable with individuals and families experiencing or have previously experienced homelessness.
  • Valid driver license and auto insurance. Driving report required.
  • Must be able to pass fingerprinting and a criminal background screening.
