Case Manager- Haleyville, AL

SCOPE 310 is seeking to hire a Support Coordinator. Full Time Position. Monday- Friday.

Essential Duties and Responsibilities:

Full time position managing a caseload and working with adults with intellectual disabilities and their families. Position will provide person centered continuity and linkage tool to effectively operate the community’s full range of services needed by the individuals served. Such services will include but are not limited to needs assessment and planning, service arrangement, social support, reassessment follow-up and monitoring.

• Responsible for the development of a person-centered plan.
• Establishes links between the individual and service providers or other community resources.
• Conducts a systematic re-evaluation of the individual's human service needs and progress.
• Completes appropriate and timely documentation and forms as required in individual's record.
• Provides service coordination services and activities on an outreach basis.

Requirements:

• Bachelor’s degree in human services field required.
• Must have a valid driver's license, have a good driving record, and be insurable.
• Must be willing to undergo a background screening.
