About the Role
The Case Manager coordinates and monitors services that help waiver participants live safely at home and in their communities. This role blends office and field work—handling assessments, documentation, and home visits—to ensure person-centered, compliant, and compassionate care. You’ll work closely with participants, families, and providers while receiving strong training and ongoing team support.

Key Responsibilities

  • Lead person-centered planning and ensure services align with Kentucky Medicaid/HCBW guidelines.
  • Complete assessments, service plans, and documentation accurately and on time.
  • Coordinate services with internal teams and external providers.
  • Conduct home and community visits to monitor progress and well-being.
  • Advocate for participant rights, preferences, and informed choices.
  • Maintain compliance with all regulations, documentation, and reporting standards.

Qualifications

  • Experience in case management, social services, or Medicaid/HCBW preferred.
  • Education in social work, human services, nursing, or related field.
  • Strong communication, organization, and problem-solving skills.
  • Comfort with technology and required documentation systems.
  • Valid driver’s license, reliable transportation, and required pre-employment screenings.
