The Case Manager works in conjunction with the Service Coordinator(s) from Local Authorities to ensure that quality services are provided to all individuals, that individuals’ needs are being met in accordance with the individuals’ and families wishes and goals, and that all federal and state rules pertaining to the individual are followed as outlined in the Home and Community-Based Standards (HCS) and Texas Administrative Codes (TACs).
- Oversees and ensure all program areas are monitored and addressed routinely Completes the development and implementation of each person’s Individual Plan of Care
- Conducts face-to-face visits with assigned service recipients at least quarterly Completes a regular review of each assigned service recipient’s progress towards outcomes as well as service utilization
- Participates in regularly scheduled meetings
- Reviews and approves consumer expenses monthly or as needed
- Coordinates Medicaid services via initial application, annual or semi-annual review for people for whom Draco is assigned Representative Payee; and, provide assistance as needed for those Draco is not Representative Payee
- Coordinates Social Security benefits for people for whom Draco is assigned Representative Payee
- Coordinates continuous integration with other intra/inter agency staff, consultants, families and guardians
- Coordinates and monitors the delivery of waiver and non-waiver services, including review of service delivery documentation
- Provides person-specific training of residential staff and contractors to ensure adequate service delivery
- Maintains positive relationship with other Draco staff and contractors to ensure continuity of care
Qualifications Required:
- Experience in social services and/or working with individuals with developmental disabilities preferred.
- High School Diploma Required
- Good interpersonal communication skills
- Knowledge of HCS preferred
- Ability to handle multiple tasks efficiently and effectively
- Excellent organizational skills
- Valid Texas Driver's License
Pay: $18.00 - $20.00 per hour
