Position Overview
We are seeking a bilingual (English/Spanish) Case Manager with home care experience to join our team. This role is centered around customer service, client support, care coordination, communication, and problem-solving.
As the primary point of contact for clients and families, you will manage client schedules, coordinate services, address concerns, maintain eligibility and authorizations, and work closely with caregivers, managed care organizations, insurance providers, state agencies, and internal teams to ensure clients receive the support they need.
This is a full-time, office-based position with a regular schedule of Monday through Friday, 9:00 AM to 4:00 PM, offering a 35-hour workweek. The role also includes periodic in-home client visits and participation in a rotating on-call schedule.
The ideal candidate is organized, accountable, positive, and able to manage multiple priorities while delivering a high level of service and support.
Responsibilities
- Manage a caseload of home care clients.
- Serve as the primary point of contact for clients, families, and caregivers.
- Coordinate client schedules, services, and caregiver coverage.
- Address client concerns and service-related issues promptly and professionally.
- Verify and maintain client eligibility, authorizations, and required documentation.
- Communicate with managed care organizations, insurance providers, and state agencies regarding client services.
- Maintain accurate records and documentation in HHA Exchange (HHAX).
- Collaborate with scheduling, recruiting, and administrative teams to ensure continuity of care.
- Conduct in-home client visits to assess well-being, service satisfaction, safety, and ongoing service needs.
- Document client visits and communicate any changes, concerns, or service needs to the appropriate parties.
- Participate in a rotating on-call schedule to support clients and caregivers outside of regular business hours.
- Ensure compliance with company policies and payer requirements.
Qualifications
- Minimum 2 years of experience in a home care agency setting required.
- Bilingual in English and Spanish required.
- Experience using HHA Exchange (HHAX) required.
- Knowledge of Medicaid, managed care plans, authorizations, and eligibility processes preferred.
- Strong communication, organizational, and customer service skills.
- Ability to manage multiple priorities in a fast-paced environment.
- Valid driver's license, reliable vehicle, current vehicle registration, and valid automobile insurance required.
