Mending Hearts is a 501(c)3 nonprofit that helps women restore their lives from addiction through a long-term treatment program that offers hope and healing in a supportive community environment – regardless of their ability to pay.

As the largest nonprofit in Tennessee offering the full continuum of care (detox through independent living) in Tennessee, we provide a holistic approach to addiction recovery through compassionate phases of care in a family environment. We identify the triggers and stressors of addiction while establishing the support needed for recovery. Our unique program develops independence and offers opportunities to reintegrate into society with confidence.

MINIMUM QUALIFICATIONS:

Must have a Bachelors’ degree in social work or related field from an approved program with 1 year experience in working with those who have been impacted by homelessness, OR high school diploma, with 3-years of experience working in employment navigation, substance use treatment navigation, and working within or with various community resource programs to enhance the quality of life for those impacted by homelessness. Ability to communicate orally and in writing, and strong computer skills. Candidate must be able to secure an F Endorsement on a valid Tennessee driver's license within three business days of start day.

GENERAL RESPONSIBILITIES:

Provides direct case management services within the community and/or the homes of those impacted by homelessness and who are seeking services to enhance their identified barriers to maintain quality of life. This includes, but is not limited to, going into the community and homes of the participants to complete a case management assessment of identified barriers and resource needs. This position is grant funded as "Mend Me" - part of the Metro / Davidson County Office of Homeless Services Housing First Supportive Services initiative.

KEY DUTIES:

· Performs With the participant and other multi-disciplinary team members, evaluates the participant, develops an individualized case management assessment and provides linkage to community resources, as well as evaluative the participants progress, documenting appropriately as they progress in the case management assessment.

· This position will require majority of time in the field to bring case management services within the homes of the participants.

· Assists the participant in making the appropriate referrals.

· Assists the Program Director in ensuring agency compliance with licensure and other regulations imposed by the government agencies involved in the program, insurance companies, contracts, etc.

· Establishes and maintains contact as requested by the referral source of each participant assigned, providing appropriate updates for them.

· Participates in community resource awareness by actively locating various community resources to enhance the quality of life by those impacted by homelessness.

· Performs other duties as requested by Program Director.

· Provides support in processing and collecting data for the HMIS database.

Job Type: Full-time

Pay: $40,000.00 - $43,000.00 per year
