Case Manager, Housing-Mental Health (TCM-Scattered Site)

Why This Role Matters

Avivo’s Ending Homelessness division provides pathways to housing for individuals experiencing homelessness through Housing First, harm reduction, and trauma‑informed approaches.

As a Case Manager, Housing – Mental Health (TCM), you will support adults experiencing serious and persistent mental illness as they transition from homelessness into permanent, scattered‑site housing. You will help clients stabilize their mental health, navigate complex systems, and maintain housing by providing consistent, community‑based support. This role is ideal for someone who is adaptable, relationship‑driven, and committed to equity and culturally responsive care.

What You’ll Do

  • Build trusting, professional relationships with adults experiencing homelessness and serious and persistent mental illness using trauma‑informed, person‑centered approaches.
  • Provide Rule 79 Adult Mental Health Targeted Case Management, including assessments, goal planning, referrals, follow‑up, and ongoing monitoring.
  • Support clients in locating, securing, and moving into scattered‑site housing that meets their individual needs.
  • Coordinate and link clients to mental health, medical, chemical health, and community‑based services to support stability and well‑being.
  • Meet regularly with clients in the community, using creative and flexible engagement strategies to support progress toward individualized goals.
  • Work collaboratively with landlords and property managers to build positive relationships and support successful tenancies.
  • Address housing‑related concerns proactively, including mediation between clients and landlords when needed.
  • Complete timely, accurate documentation and required paperwork in electronic systems to ensure compliance and quality care.
  • Respond to and consult on safety concerns, crises, and mandated reporting situations when they arise.
  • Actively engage as a team member, participating in meetings, supervision, and Avivo’s commitment to undoing racism and reducing disparities.

Schedule

  • Full‑time, non-exempt role with a 32‑hour workweek, supporting strong work‑life balance.
  • Typically Monday–Friday, daytime hours, with flexibility to meet program needs.

What You Bring

Required

  • Bachelor’s degree in a human services‑related field
  • 2,000+ hours of supervised experience working with adults with mental illness
  • Eligibility to provide Rule 79 Adult Mental Health Targeted Case Management per DHS guidelines
  • At least one year of experience working with single adults experiencing homelessness or similar populations
  • Valid driver’s license, access to a personal vehicle, and ability to travel throughout the community
  • Flexibility to work occasional evenings or weekends to meet participant needs
