POSITION PROFILE:
Do you understand the impact housing-related issues can have on an individual and family health and well-being? Are you passionate about actively assisting others to address barriers to stable housing, facilitate access to housing resources, and navigate challenging systems? If you answered “YES”, it sounds like the SAY Neighborhood Networks Housing Navigator position would help you achieve that goal! Housing Navigators provide housing related community supports to clients with identified housing needs through coordination with Neighborhood Navigators and ECM services. Main duties include conducting personal assessments, guiding individuals in developing housing care plans and goals, and connecting individuals to resources and support in the community, all with the goal of providing housing tenancy and sustaining services.
Program hours: Monday – Friday 7 A.M. to 7 P.M (not to exceed 8-hour days). Hybrid work scheduled available not to exceed 3 days per week remote work. Bi-lingual English/Spanish speaker preferred.
ESSENTIAL JOB FUNCTIONS AND RESPONSIBILITIES:
- Provide case management services through client-centered approaches such as Motivational Interviewing and strength-based interventions.
- Manage case load of 35-40 members to include intake, client needs assessment, goal setting, develop individual service plans, advocacy for client development, and follow-up services.
- Maintain accurate client files, document case notes in timely manner, and complete funder reports as requested.
- Schedule and conduct home or in-person visits with clients for intake and or client servicing appointments.
- Coordinate and conduct group facilitation may include trauma informed services, leadership development, socio-emotional curriculum, or other topics of interest.
- Develop and maintain positive relationships with clients and other connected program partners,
- Identify and remain knowledgeable of program related resources: self-sufficiency benefits, housing related resources and interventions, employment opportunities, financial literacy, healthcare and behavioral health services.
- Contact eligible members to assist in development, implementation and evaluation of Comprehensive Assessments, Social Determinants of Health Assessments, and Housing Assessments.
- Advocate on behalf of clients with landlords and other housing professionals, work with property management personnel to ensure well supported care needed to maintain housing eligibility.
- Actively recruits program participants/clients using referral resources and community engagement.
- Attend and participate in partner and funder-related meetings.
QUALIFICATIONS (Education, Experience and Certifications):
- Bachelor’s Degree in Public Health, Social Work, Human Services, or related field preferred
- Experience in case management, group facilitation, direct client service, and delivering curriculum-based interventions with youth and families.
- Lived or Allyship experience working with complex physical, behavioral, or developmental health needs, individuals at risk of or experiencing homelessness, and/or individuals transitioning from incarceration.
- Bi-lingual English/Spanish preferred.
