Case Manager (I/DD Waiver)

Position Summary:

We are seeking a passionate, organized, and self-motivated Case Manager to join our growing team. This full-time position will manage a flexible caseload within the Kanawha County, Putnam County, and Cabell County areas. The ideal candidate is an excellent communicator, highly dependable, and values making a positive impact in the lives of others.

Key Responsibilities:

• Coordinate and facilitate IDT meetings, annual IPPs, and Critical Juncture reviews
• Conduct monthly home visits and ensure compliance with all IDD Waiver requirements
• Advocate for individuals by identifying unmet needs and submitting appropriate documentation
• Monitor services and supports to ensure quality, safety, and satisfaction
• Collaborate with providers, families, and other stakeholders
• Maintain accurate documentation and submit reports within required timelines
• Remain up-to-date on policy changes and attend required training sessions

Qualifications:

• Must meet WV requirements to provide case management services under the IDD Waiver
• Bachelor’s degree in human services, social work, psychology, or a related field required
• Previous experience with IDD Waiver services strongly preferred
• Reliable transportation and willingness to travel regularly to Kanawha County, Putnam County, and Cabell County
• Strong communication, organization, and time-management skills
• Must pass background checks and maintain required certifications
