Case Manager II
People Incorporated is seeking a Case Manager II to join our Waiver Case Management Dakota County team to work with clients who are diagnosed with chronic mental illness. We support people in connecting with resources to live as independently as possible. We help them develop self-awareness to make positive plans for the future through frequent meetings in the community. Case Managers develop long term relationships with their clients and operate autonomously, creating their own schedules. Waiver Case Management involves more behind-the-scenes work supporting client needs than time meeting with clients. We value work-life balance and have manageable caseloads that are around 30 clients on average!
Hours: Full-Time, Monday-Friday, 8am-4pm with flexible business hours
Location: Dakota County, Minnesota | Hybrid with remote opportunities
Range: $24.25 - $25.75/hourly based on qualifications
Hiring Bonus: $1,000 to external candidates
