Since 1952, Light of Life Rescue Mission has been a safe refuge for homeless and hurting people in the Pittsburgh area.
We currently seek a full-time Case Manager (Level 2) for the WomenChildren's Longterm Program. This person will walk closely alongside clients in guiding them through a four-phase recovery program that combines clinical and spiritual elements. Successful people in this role tend to be persuasive problem-solvers who are skilled in building relationships and inspiring others.
Responsibilities include but are not limited to:

Provide case management and advocacy for clients, working as a liaison to coordinate wraparound services, while making referrals for housing, healthcare, and other resources for families.

Develop individualized treatment plans addressing substance use and co-occurring disorders and teach families about addiction, recovery, trauma, and effective support strategies. Act as a liaison between medical and social services providers as needed.

Maintain detailed, confidential client records and case notes. Track clients' care plan progress through one-on-one discussions and using the online client management system, MissionTracker.

Provide immediate support during emergencies and connect clients to appropriate crisis services.

Network with related service providers to access appropriate services for clients.

Serve as a treatment team member, providing input and support in Care Team decisions.

Make decisions about proposed activities in accordance with Light of Life principles and the best practices for recovery.

Transport clients in the Mission's vehicles to and from appointments as needed.

Work in tandem with the children's case manager and volunteers.

Education/Experience:
-
Bachelor's degree required in social work, counseling, psychology, human services, or another related field. A Master's degree is preferred.

5-years of experience working with individuals in a substance recovery or mental health setting.

Excellent customer service experience.

Valid driver's license and clean driving record.

HMIS experience and knowledge preferred.

Proficient in Microsoft Office.
