Case Manager II - Women & Children's Longterm Program

Since 1952, Light of Life Rescue Mission has been a safe refuge for homeless and hurting people in the Pittsburgh area.

We currently seek a full-time Case Manager (Level 2) for the WomenChildren's Longterm Program. This person will walk closely alongside clients in guiding them through a four-phase recovery program that combines clinical and spiritual elements. Successful people in this role tend to be persuasive problem-solvers who are skilled in building relationships and inspiring others.

Responsibilities include but are not limited to:

  • Provide case management and advocacy for clients, working as a liaison to coordinate wraparound services, while making referrals for housing, healthcare, and other resources for families.
  • Develop individualized treatment plans addressing substance use and co-occurring disorders and teach families about addiction, recovery, trauma, and effective support strategies. Act as a liaison between medical and social services providers as needed.
  • Maintain detailed, confidential client records and case notes. Track clients' care plan progress through one-on-one discussions and using the online client management system, MissionTracker.
  • Provide immediate support during emergencies and connect clients to appropriate crisis services.
  • Network with related service providers to access appropriate services for clients.
  • Serve as a treatment team member, providing input and support in Care Team decisions.
  • Make decisions about proposed activities in accordance with Light of Life principles and the best practices for recovery.
  • Transport clients in the Mission's vehicles to and from appointments as needed.
  • Work in tandem with the children's case manager and volunteers.

Education/Experience:

  • Bachelor's degree required in social work, counseling, psychology, human services, or another related field. A Master's degree is preferred.
  • 5-years of experience working with individuals in a substance recovery or mental health setting.
  • Excellent customer service experience.
  • Valid driver's license and clean driving record.
  • HMIS experience and knowledge preferred.
  • Proficient in Microsoft Office.
