PROGRAM OVERVIEW
The Individuals Rapid Rehousing Case Manager is responsible for supporting program participants in developing housing stability plans and addressing barriers to housing. To this end, the Individuals Rapid Rehousing Case Manager participates in our collective effort to end and prevent homelessness.

REPORTING RELATIONSHIP: The position reports to the on-site Program Manager and is a vital member of the participant’s care team.
