Case Manager (Intensive) - Family Connect

The Carroll County Youth Services Bureau is a non-profit organization that has provided outpatient mental health services in Carroll County since 1972. CCYSB has grown into an agency that commits itself to providing a full continuum of mental health and substance use services for children, families, and adults in Carroll County.

Summary

The Intensive Case Manager for Family CONNECT will provide comprehensive assessment and delivery of in-home case management and wrap around services to Carroll County families with youth identified through Carroll County Public Schools. The Intensive Case Manager will work collaboratively on a team with therapeutic staff. The Intensive Case Manager will remain up to date on community resources and provide supportive work to families in their homes. This may include working with families to provide referrals to community resources, providing concrete needs, psychoeducation, and teaching communication or problem-solving skills.

Essential Functions

  • Provide on-going and in-home support and case management services to Carroll County families with children in the Family CONNECT program identified through Carroll County Public Schools.
  • Develop meaningful and appropriate case management plans for all clients.
  • Monitor client’s progression through their case management plan and revise plan as needed.
  • Remain up to date with community resources and provide information and referrals to clients as appropriate.
  • Provide supportive work with individual/family subsystems including but not limited to self-care, psycho-educational information, role modeling and anger management.
  • Provide coordination of care services for all clients as indicated.
  • Provide seven days of 24/7 emergency on-call coverage on rotational basis.
  • Provide supportive group work, as needed, through psycho-educational and support groups.
  • Transport clients to scheduled appointments and services related to assessed needs.
  • Maintain written and/or electronic case records and contact documentation as specified per agency, policy, procedure and grant requirements.
  • Conduct crisis intervention as needed.
  • Attend scheduled administrative supervision meetings.
  • Attend weekly treatment team meetings and monthly agency staff meetings.
  • Attend scheduled clinic consultation.
  • Work collaboratively with other Family CONNECT (FC) staff.
  • Maintain program data collection and complete quarterly reports for Local Management Board

Knowledge Required

  • Bachelor’s degree in mental health field such as social work, psychology, or human services
  • One or more years of case management or clinical experience with families and children
  • Working knowledge of case management systems and planning techniques.
  • Basic working knowledge/familiarity with Carroll County community resources (preferred)
  • Bilingual (preferred)
  • Ability to operate computer programs such as the Office 365, Email, and Internet. SPSS, and electronic health record program preferred
  • Ability to use general office equipment (computer, phones, copiers, fax, etc.)
  • Ability to use initiative and judgment in scope of designated responsibilities to ensure that matters requiring attention are appropriately delegated or handled
  • Able to effectively communicate with clients, vendors, contractors, supervisors, co-workers, and other persons in writing and by telephone while remaining professional, pleasant, and respectful
  • Ability to conduct oneself as a team member through use of collegial words and actions that support co-workers and Carroll County Youth Service Bureau’s philosophy, mission and vision.

Physical Working Conditions

Office

  • When working onsite this job requires sitting the majority of the time; lifting/carrying/moving objects up to 10 pounds; bending or reaching to file and working while standing/moving
  • Smoke-free office and property
  • Lighted, heated/cooled area typical of an office
  • Noise from office equipment including telephone, computer, copier, etc. and from staff interactions.

Off Site

  • When working off-site employee may encounter homes/buildings in the community that may be smoke-filled and non-handicap accessible.
  • Lighting, heating/cooling may vary greatly.
  • Noises typical in a family home environment including television, radio, telephone, children, pets, and other adults.
  • Employees may be exposed to common household allergens, including pet dander, dust, pollen, and other environmental irritants.
  • The cleanliness and overall condition of homes and community settings may vary, and employees should be prepared to work in a range of environments while maintaining professionalism and adhering to agency safety guidelines.

Licensing

  • Valid driver’s license with no more than two points

Training

Must participate and successfully complete a variety of training which may include Infection Control, HIPAA, Corporate Compliance/Ethics, Consumer Rights, First Aid/CPR and other training as requested.

Pay: $37,000.00 - $42,000.00 per year
