Overview
We are seeking a dedicated and compassionate Case Manager Intern to join our team. This paid internship offers an excellent opportunity for individuals interested in behavioral health, social work, and community support services. The intern will gain hands-on experience assisting clients with various needs, including crisis intervention, immigration support, benefits support, and child welfare. This role is ideal for those passionate about making a positive impact in the community while developing essential skills in case management and social services. The Case Manager Intern will support the delivery of client services by assisting with screenings, referrals, and follow-ups under the guidance of the Program Manager.

Duties
- Assist in conducting screenings to assess clients' health and social service needs.
- Support the development of individualized service plans and help connect clients to resources (e.g., WIC, SNAP, housing, healthcare).
- Help schedule and track client appointments, referrals, and follow-ups.
- Maintain accurate and confidential client records under supervision.
- Collaborate with internal staff and external partners to ensure continuity of care.
- Monitor client progress and report observations to the Program Manager.
- Participate in team meetings and training sessions.
- Perform other duties as assigned to support program goals.
- Engage in community outreach activities to promote available services
- Collaborate with team members to ensure comprehensive client support and service delivery
Requirements
- Currently enrolled in or recently graduated from a program in social work, public health, human services, or a related field.
- Strong interest in case management and community-based services.
- Excellent organizational and communication skills.
- Ability to work collaboratively and maintain confidentiality.
- Commitment to equity, inclusion, and trauma-informed care.
- Haitian Creole, Spanish fluency

Internship Details
- Duration: 4 months
- Schedule: Part-time, 9:00 AM – 3:00 PM, with some flexibility required for events.
