Case Manager Maternal Group Home

Title: Case Manager - Maternal Group Home

Status: Full Time, Non-Exempt

We are seeking a Case Manager to provide trauma-informed, youth-centered case management services to pregnant and parenting youth ages 16 to 20 in a Maternal Group Home setting. This role supports young families by coordinating healthcare, housing, education, employment, and supportive services to promote safety, stability, and long-term success.

Key Responsibilities
  • Conduct comprehensive assessments and identify client strengths, needs, and goals
  • Develop and maintain individualized service plans in collaboration with clients
  • Connect participants to healthcare, behavioral health, housing, education, childcare, and employment resources
  • Provide advocacy to remove barriers and ensure timely access to services
  • Coordinate care with internal staff and community providers
  • Support transition planning, aftercare, and follow-up services
  • Monitor client progress and adjust service plans as needed
  • Maintain accurate and timely documentation in HMIS and other required systems
  • Participate in team meetings, case conferences, and supervision
  • Respond to crises and ensure client safety

Qualifications
  • Bachelor’s degree in Social Work, Psychology, Human Services, or related field
  • 1-3 years experience in case management or direct client services strongly preferred
  • Experience working with youth, young adults, or families preferred
  • Knowledge of trauma-informed care and Positive Youth Development preferred
  • Strong advocacy, communication, and organizational skills
  • Experience with HMIS or similar systems a plus
