Qualifications and Responsibilities of Senior Care Case Manager

3+ years experience int the field
Desire to improve quality of life for aging adults.
High level of commitment, dependability, and interpersonal skills.
Prior experience in a nursing or long-term care setting is preferred.
Prior experience in social services, human services, or a related field is preferred.
Valid driver’s license and reliable transportation with current automobile insurance meeting state minimum liability requirements.
Computer skills are required for documentation and system use.
Associate's degree preferred
