Your proactive approach and expertise will help navigate complex situations, ensuring clients receive the resources and guidance they need to move forward confidently.
Duties and Responsibilities:
- Conducts initial screening of shelter applicants and provides crisis intervention and problem-solving support to first attempt to divert households from entering Shelter.
- Creates Individualized Housing Plans (IHP) for each Guest in the shelter to assist them in their weekly goals, providing information, referral, and advocacy to help access programs for assistance for housing placement and stabilization.
- Assists guests in identifying housing barriers and supports them to create goals to that address and overcome barriers to obtain permanent housing
- Facilitates appropriate referrals and warm hand offs to community partners and organizations
- Utilizes trauma-informed care and other evidence-based approaches for engaging and supporting guests with their goal plans and with meeting expectations of shelter residency.
- Adheres to established program policies and procedures, including strict adherence to Guest confidentiality.
- Assists with completing program reports for MMHA management, funders, etc., as needed and requested.
- Establishes and maintains case files including relevant case file documentation such as Guest screening/eligibility documentation, Guest intake, assessment, and case plan records, case notes, Guest consent forms, etc.
- Maintains knowledge of all relevant federal, state assistance programs and related community programs and services.
- Collects and manages data necessary for program monitoring and reporting and consistent with agency and state policies and procedures for Homeless Management Information System (HMIS).
- Engages in ongoing professional education (e.g., remote workshops, conferences) to learn about and adopt emerging and established evidence-based practices.
- Other duties as assigned.
Skills and/or Experience:
- Bachelor’s Degree in Social Work or a related discipline preferred.
- At least two years’ experience providing trauma-informed case management or
- similar services for people who are severely economically disadvantaged and have complex service needs.
- Computer literacy with knowledge of Microsoft Office Suite software with excellent word processing and spreadsheet skills.
- Excellent problem resolution, influencing, and conflict management skills.
- Ability to work independently within the Shelter Program and in the broader Medina County community to meet the needs of Shelter guests.
- Superior communication skills for guest, staff, and stakeholder interaction.
- Must be certified in CPR/First Aid or able to attain certification.
Inherently Hazardous and/or Physically Demanding Working Conditions:
- The essential functions of this job require the encountering of irate clients or individuals and exposure to environmental factors including cold, excessive heat, noise, fumes, dirt, contagious diseases, unsanitary conditions, insect infestation, bodily wastes, odors, common office chemicals, and may involve lifting of up to 40 pounds.
- This position’s incumbent may be required to transport clients and/or make home visits post Shelter exit.
Additional Requirements:
- Must successfully undergo a criminal background check
- Included in the on-call rotation as needed for staffing coverage
- Must possess a valid Ohio Driver’s License and be acceptable to the Authority’s
- automobile insurance provider
- Must provide proof of eligibility to work in the United States
- Must be able to deal with culturally diverse populations and perform within strict ethical standards
- This position may require additional training and travel in order to meet performance expectations
- Maintain strict confidentiality of client issues and agency matters as defined by professional ethics and applicable laws.
