Role Description
This is a flexible role for a Case Manager, located in Georgia, with requirements to travel within west, north, and east GA. The Case Manager will be responsible for assessing, planning, and implementing care plans for clients. Day-to-day tasks include liaising with clients and their families, coordinating with service providers, monitoring client progress, maintaining detailed records, and ensuring that clients receive the necessary services to meet their individual needs. The Case Manager will work closely with other team members to enhance client outcomes and ensure continuity of care.

Pay: $15.00 - $25.11 per hour
