Case Manager (NY Aging)

Summary:

The Case Manager oversees the application/Intake/Information gathering process, oversees the case management system, coordination of program services, and the community referral process; developing Individualize Service Plans (ISP)/ care plans in addressing comprehensive Older Adults member’s needs.

Responsibilities:

  • The Case Manager will complete bio-psychosocial health assessments, progress checks, Individualized Service Plans (ISP), care plan development, goal setting, treatment planning, develops intervention plan that addresses identified need for recovery, housing, employment, substance abuse treatment, mental health coordination, and ISP implementations, arrangements of client services, on-going monitoring of clients wellbeing, linkages to mental health services, linkages to permanent housing, and other support services as needed.
  • Maintains intensive Case Management documentation standards for all program cases, completes progress notes, and ensures daily case documentation and computer case note entry into Computer Case Management System.
  • Ensures/maintains confidentiality of client files, program, and HIPAA information.
  • Maintains updated community resources for utilization of coordinating services, providing/dispersing information, referrals or consultations as needed.
  • Assist with other work related tasks as deemed appropriate for effective operations of COPO upon request of the CEO/COO OR other Supervisors of Council of Peoples Organization
  • The case managers receive information from hospital case managers, home health care companies, physician’s offices, social workers and other health care providers.
  • Case Manager’s goal is to make sure clients are getting medically necessary care, quality care, and that the care is being delivered as efficiently and economically as possible.
  • They anticipate the patient’s future health care needs and try to put in place mechanisms to meet those needs as efficiently as possible through contacting hospitals and medical staff as needed.
  • Additional detailed responsibilities include but are not limited to the following: Home visits to 6-10 clients a month 2 month monitoring calls Assessment summaries Linkage to benefits & entitlements
  • Referrals to appropriate agencies
  • Meeting DFTA standards for Older Adult Case Management

Qualifications and Requirements:

  • Bachelor’s degree required
  • Two years of related case management experience preferred.
  • Spanish Speaker Required
  • Proficiency in modern business communications including personal computers, email, voicemail, facsimile and copier equipment, client service/care.
  • Up to date Medical Examination
  • Documentation of Tuberculin free condition
  • Background check

Job Type: Full-time

Pay: $30.00 per hour
