Position Summary

The Case Manager – Social Determinants of Health (SDOH) provides comprehensive, therapeutic case management services to youth and families. This role focuses on identifying and reducing barriers that impact stability, health, and economic mobility.

Using trauma-informed and strength-based approaches, the Case Manager supports participants in overcoming challenges related to housing, food access, employment, education, and healthcare—while promoting long-term self-sufficiency and improved well-being.

Key Responsibilities

Therapeutic Case Management

  • Conduct biopsychosocial assessments and SDOH screenings

  • Develop and implement individualized service plans with measurable goals

  • Provide ongoing support using trauma-informed, strength-based practices

  • Conduct regular participant check-ins (in-person, virtual, or community-based)

  • Coordinate care with schools, therapists, medical providers, and community stakeholders

  • Monitor progress and adjust plans as needed

  • Maintain accurate and timely documentation in EHR systems (e.g., Salesforce)

SDOH Navigation & Support

  • Assess and address needs related to:

    • Housing stability

    • Employment and income

    • Food access

    • Transportation

    • Healthcare access

    • Education and workforce development

  • Connect participants to community resources (SNAP, TANF, Medicaid, rental assistance, etc.)

  • Advocate to reduce systemic barriers

  • Provide crisis intervention and stabilization services

Financial Education & Economic Mobility

  • Support financial literacy efforts (budgeting, credit building, banking basics)

  • Assist with workforce readiness (resume building, job search, interview prep)

  • Help participants set and achieve financial goals

  • Track financial outcomes and progress (HMIS or similar systems)

Collaboration & Systems Coordination

  • Partner with internal teams and external service providers

  • Participate in case staffing and interdisciplinary meetings

  • Facilitate communication between stakeholders

  • Ensure compliance with agency policies and grant requirements

Additional Duties

  • Attend staff meetings, trainings, and professional development opportunities

  • Represent GASA at community events and partnership meetings

  • Perform other duties as assigned

Qualifications

Education:

  • Master’s degree in Social Work, Counseling, Psychology, Human Services, or related field

  • Provisionally Licensed candidates preferred

Experience:

  • Minimum 2 years of experience in case management, behavioral health, or community-based services

  • Experience working with high-risk youth and/or families preferred

  • Knowledge of Social Determinants of Health and community resources

Skills & Competencies:

  • Strong organizational and documentation skills

  • Excellent written and verbal communication

  • Ability to work independently and collaboratively

  • Technologically proficient and adaptable

Additional Requirements:

  • Valid driver’s license, reliable transportation, and auto insurance

  • Ability to travel between multiple sites within the St. Louis area

  • Knowledge of budgeting, fiscal management, and grant compliance
