Seeking Case Manager to support families and individuals dealing with the challenges of aging and/or illness. This is both an interesting and rewarding job working with a very deserving population. The successful candidate will take responsibility for the full patient care cycle including: assessing, planning, implementing, monitoring and evaluating actions required to meet the client’s health and human services goals.
This job offers the use of a company laptop, to better support your work in the community. Additionally, Care Managers receive ongoing training and support.

Responsibilities:
- Coordinating and providing care that is safe, timely, effective, efficient, equitable, and client-centered
- Handling case assignments, drafting care plans, reviewing case progress and determining next steps for success
- Helping clients be safe, happy and healthy for as long as possible.
Duties:
- Provide comprehensive assessment of older adults’ physical and mental health, home safety, social needs, cultural needs, legal and financial status.
- Provide services in a culturally and linguistically appropriate manner.
- Develop care plans in collaboration with clients and help clients achieve the goals outlined in the care plans.
- Serve as a client advocate. Contact clients’ healthcare providers, community agencies, etc. to ensure client’s needs are met.
- Provide information, assistance, and referral to senior clients and their families.
- Provide ongoing oversight of each assigned case, maintaining regular contact with client (including home visits, telephone checks, annual reassessments, etc.).
- Coordinate care for clients when multiple service providers are involved.
- Maintain accurate and complete documentation (both paper and electronic files).
- Comply with all contractual requirements regarding service provision and documentation.
- Participate in community outreach activities to other professionals as well as to potential clients and their families.
Conduct care-planning assessments to identify needs
- Create and execute care plans
- Screen, arrange and monitor vendors (care agencies, lawyers, gardeners, movers…)
- Act as a liaison for out of area families
- Assist with moving clients to or from different care settings
- Provide crisis intervention
- Provide client and family education
- Act as advocate for clients at medical and other appointments
- Identify agencies and/or social services and other programs that client can avail
- Maintain accurate notes, client files and billing
- Facilitate multiple care aspects (case coordination, information sharing, etc)
- Help patients make informed decisions by acting as their advocate regarding their clinical status and treatment options
- Develop effective working relationships and cooperate with medical teams throughout the entire case management process
- Arrange alliances with social services, health and governmental agencies
- Go the extra mile and interact with patients to keep track of their progress and to ensure satisfaction
- Record cases information, complete all necessary forms accurately and produce statistical reports
- Promote quality and cost-effective interventions and outcomes
- Assess and address motivational and psycho-social issues
- Adhere to professional standards as outlined by protocols, rules and regulations
- Other duties as assigned
Education and Experience Required:
- Bachelor’s degree and two years or more of paid social service experience preferred . Case management experience preferred
- Experience conducting assessments, creating care plans, identifying and planning for appropriate services
- Knowledgeable of social services or human services issues, especially dealing with older adults and unpaid caregivers
- Knowledgeable of community resources for older and disabled adults and unpaid caregivers

Compensation:
- $50-$65/hr dependent on experience and education
- Unfortunately, we can NOT offer medical/healthcare insurance at this time.
- We offer a retention bonus of $500 after 6 months of continuous employment and $1000 after 1 year of continuous employment. *Applies to regular FT employees, pro-rated for part-time
Qualifications:
- Bachelor’s Degree required (BSW, MSW, LPN or RN preferred but not required)
- Home Care, Senior Care, or Case Management experience preferred
- Fully vaccinated for Covid-19 and proof required.
- Background in scheduling and care coordination helpful
- Must successfully pass mandatory background checks
- Valid WA State driver’s license and current automobile insurance; willing to drive personal vehicle to home visits, trainings and meetings as required
- Must be able to travel locally, 75% of the time.
- Must be able to read, write, and communicate fluently in English, additional language skills appreciated.
- Ability to provide effective and respectful quality care and services that are responsive to diverse cultural beliefs and practices.
- Regularly required to walk; use hands and arms to reach and operate objects, tools, or controls.
- Must be able to lift up to/or move up to 30 pounds.
- Specific vision abilities required by this job include close vision and the ability to adjust focus.
- Significant work will be performed offsite (e.g. home visits or external meetings).
- The noise level in the work environment is moderate.
- Proficiency navigating a personal computer and in the use of the Microsoft Office and other relevant software for word processing, calendaring, e-mail, and data entry. Efficiently research and perform various tasks via the internet.
