Case Manager-Outpatient - Bilingual
The salary range for this position is SummitStone’s good faith belief and reasonable estimate of the range of possible compensation for this position at the time of the posting. Responsible for coordinating integrated (i.e. substance abuse and mental health) case management services for our adult or children's populations. Case managers engage, assess, plan, advocate, coordinate and link clients to formal/informal activities, services, and/or support within the community that promote wellness and safety. Case managers provide individual, group, family, and crisis therapeutic interventions. Case Managers develop and monitor service plans, document interactions, maintain clinical records, and other necessary data reporting for adult and child consumers/clients with co-occurring disorders and/or disabilities in Larimer County. Case managers coordinate not only with external collaterals in the community, but internal treatment and medical teams as well. Position will include individual orientations and coordination of our SUD IOP program.
