Position Overview
New Directions Addiction Recovery Services (NDARS) is seeking a compassionate and motivated Case Manager to support residents in achieving stability, independence, and improved quality of life. This role involves working closely with individuals to assess needs, develop personalized plans, and connect them with critical community resources.
Key Responsibilities
- Meet with assigned residents at least bi-weekly to assess needs and progress
- Conduct intake assessments and develop individualized action plans
- Monitor and track client progress using electronic case management systems (EHR)
- Connect clients to community resources, including employment, education, and recovery services
- Facilitate goal setting and support self-management strategies
- Develop behavior contracts to address ongoing challenges
- Administer drug screenings as needed
- Maintain professional communication with clients, staff, and community partners
- Attend trainings, meetings, and community events as required
Qualifications
Required:
- Must be 18 years or older
- Valid driver’s license, reliable transportation, and clean driving record
- Familiarity with 12-step recovery principles
- Strong interpersonal and communication skills
- Ability to work with diverse populations
- Proficiency in Microsoft Office or Google Suite
- Ability to pass background check and drug screening
Preferred:
- Personal or professional experience in substance use recovery
- Experience with alternative recovery modalities
- Previous case management or recovery coaching experience
- Background in public service or human services
- Spanish language skills (a plus)
Education
- High school diploma or equivalent required
- Additional certifications in substance use disorder or related fields are highly valued
Schedule & Work Environment
- Flexible schedule required
- Some on-call responsibilities between 8:00 AM – 11:00 PM
- Combination of structured and on-demand hours
