Reporting to the Clinical Supervisor, the Case Manager provides culturally responsive, client-centered care as part of a multidisciplinary team for individuals with complex mental health and psychosocial needs.
This role includes short-term post-crisis follow-up to support stabilization, reduce crisis recurrence, and connect clients to ongoing services.
The Case Manager will assist with referrals, care coordination, and system navigation across multiple systems, including hospitals, crisis services, housing programs, legal systems, schools, and medical providers

Manage assigned caseload while delivering highquality, clientcentered care

Provide 5+ hours of direct services daily, including outreach and engagement

Conduct rapid followup and ongoing support for individuals postcrisis discharge
-
Deliver fieldbased services in homes, shelters, hospitals, and community settings

Assess risk, needs, and barriers; develop shortterm stabilization plans

Coordinate care and link clients to mental health, medical, and substance use services

Facilitate appointments and warm handoffs to ensure continuity of care

Collaborate with multidisciplinary teams and external partners

Support clients in accessing housing, food, transportation, ID, and public benefits

Advocate to reduce systemic barriers and improve service access

Coordinate across systems (healthcare, legal, housing, social services)

Engage families and natural supports to strengthen care networks

Provide culturally responsive, traumainformed services

Maintain accurate documentation and full regulatory compliance

Support successful transitions to ongoing care and reduce crisis reentry

Bachelor’s degree in social, behavioral, or human services field required.

Must obtain Washington State Agency Affiliated Counselor credential.

Experience working with highacuity populations, including individuals experiencing crisis, homelessness, or complex behavioral health needs preferred.

Knowledge of behavioral health systems, crisis response systems, and community resources in King County.

Strong care coordination, advocacy, and system navigation skills.

Ability to work in communitybased settings with a high level of independence and accountability.

Understanding of traumainformed care and recoveryoriented practices.

Demonstrated ability to provide culturally responsive services.

Bilingual (English/Spanish) preferred.
-
Valid driver’s license, reliable transportation, and acceptable driving record.

Proficient in Microsoft Office and electronic health records.
This Full-time in-person position works a typical schedule of 8 AM - 5 PM, Monday to Friday.
$ 27.00 to $ 32.45 an hour, depending on experience, qualifications, and credentials.

At that time, there were no Latino-based behavioral health care organizations in WA State. Consejo became a pioneer in establishing the first bilingual and bicultural care organization dedicated to serve the Spanish speaking monolingual community. The mission was to improve mental health care access and break down cultural stigmas. Over four decades later, we continue to lead the way in providing culturally and linguistically competent behavioral health services to the Latinx community, empowering and transforming the lives of our clients.
