The Case Manager/QIDP will work at our community day services program where program participants engage in on-site and community-based recreational and educational activities of the program participants' choosing with a few individuals engaging in work activities. The case manager will work with a team to assist individuals with developmental disabilities in achieving their personal goals through advocacy, guidance, and coordination with co-workers and other service entities.

REQUIREMENTS
- Bachelor's Degree in Human Services, Psychology, Sociology, Counseling, Social Work, Education, or a related field (Please feel free to contact us if you are unsure whether your degree will qualify.)
- Valid/Current Driver's License
- One Year of Experience Working with Individuals with Developmental Disabilities (Note: Experience can be personal, professional, intern/practicum, volunteer-related, or a combination of each.)
- Must pass a criminal background check, as required by the Healthcare Worker Background Check Act (225 ILCS 46), and a DCFS Child Abuse and Neglect background check
- Successfully complete and maintain American Heart Association or American Red Cross CPR/First Aid certification
SUMMARY OF DUTIES
- Communicating with families, program participants, and other service providers to coordinate day program services and to ensure that the individuals needs are met.
- Responsible for writing service plans and daily/monthly note summaries in addition to related paperwork to facilitate services.
- Responsible for the day-to-day supervision and/or coordinating supervision of the day program participants.
- Assist in planning/organizing community-based activities for program participants based on their expressed interests.
- Must be timely and accurate in completing documentation, including daily attendance for the day program participants.
- Must be committed to improving the lives of individuals with developmental disabilities.
SCHEDULE
- 40 hours per week
- Monday through Friday, from 8 am - 4:30 pm (hours may vary slightly, depending on bus/transportation schedules)
- Please note that our day programs are closed on weekends and most major holidays.
WAGES
- $21 per hour with a $0.50 increase after training is completed
