Case Manager - Mental Health Skill Building
Description:
Responsible for development and implementation of individualized service plans in accordance with program.
Responsibilities:
- Provide direct services to clients, including linkage, advocating, supporting, and providing education as needed.
- Prepare written individual service plan, incorporating input from clients, involved service providers, and clearly identifying problem areas and needs, strategies, and service objectives.
- Link clients with external programs or services, such as health services, recreational activities, childcare services, financial resources, employment resources, childcare, AA/NA groups, transportation resources, and others
- Collaborate and coordinate with other service providers and professionals, such as attorneys, social workers, and probation/parole officers.
- Complete all progress notes and quarterly reports in a timely fashion.
- Recommend appropriate services for client.
- Assure client is informed of his/her human rights.
- Maintain weekly contact with referring agency.
- Conduct discharge planning in accordance with program description.
- Attend all meetings as required.
- Provide accurate time sheets.
Qualifications (Required)
Experience:
Qualified Mental Health Professional (2 years)
Education:
Bachelor's (Psychology, Human Services, Social Work)
License:
QMHP-A
First Aid and CPR certification
Crisis Wave/TOVA certification'
